Challenge FC Registration Process
After a player has accepted a position on a Challenge FC team, each player must be registered. Please see the Parent Checklist below to ensure all steps are followed. Please note all paperwork MUST be completed and turned in to their team manager. If it is incomplete, a player card cannot be issued and could result in delay of playing.
There are SIX steps to the registration process. All players returning and new will have to complete these steps.
Parent Registration Checklist
STEP 1: AYSO Player Registration/Concussion Form — Complete the online registration at your AYSO Region’s Website. For example: www.ayso688.org. Print off a copy. Make regional payment online. All other CFC payments are made to your team manager or treasurer.
For all others, go to your individual region's site.
STEP 2: Challenge FC Membership/Medical Release Form (updated May 2018) — Signed by parent and player (notarized if playing out-of-state)
STEP 3: ASA Head Injury/Concussion Policy & Procedures (updated 2015) — Signed by parent and player
STEP 4: ASA Staff/Parent/Player Code of Conduct (updated 2017) — Signed by parent and player
STEP 5: Birth Certificate — Show original for verification and provide a copy (check with team if you are a returning player)
STEP 6: 1 x 1 Photo — for ASA Player Pass (check with coach if required)
